By referring patients in need of wound care services to Rapid Community Health Care, we can collaborate to provide high-quality treatment and improved healing outcomes—delivered directly to where the patient resides.

To begin processing the referral, please submit the following:

• Face Sheet / Patient Demographics

• Signed Physician / PA / NP Order for Wound Care

• Insurance Information

• (Optional) Family Member Contact Info — for scheduling coordination

📁 Other Helpful Documentation

These additional documents help us assess and treat the patient more quickly and effectively:

• Wound Photos (if available)

• History & Physical (H&P)

• Recent Progress Notes

 

For Surgical Wounds Created Within the Last 90 Days:

A surgeon’s order is required to approve Rapid Community Health Care to evaluate and treat, due to the 90-day global surgical period.
